Cervical Dilation and Effacement: What the Numbers Mean
As labour approaches, the cervix changes from firm, long and closed to softer, thinner and open. Dilation describes opening in centimetres; effacement describes thinning and shortening, often recorded as a percentage. These findings help your care team understand what your cervix is doing, but they are not a countdown clock. People can remain slightly dilated for days or change quickly during labour.

Dilation: from closed to fully open
Dilation is the width of the cervical opening, measured from closed to about 10 centimetres. The NHS describes established labour as the point when contractions are stronger and regular and the cervix is around 4 centimetres dilated; definitions can vary between services. At about 10 centimetres the cervix is considered fully dilated, marking the beginning of the second stage when the baby is born.
Effacement: the cervix becomes thinner
Before pregnancy the cervix has length and thickness. Effacement means it shortens and thins as it prepares to open. In systems that use percentages, 0% means little thinning and 100% means fully effaced. Some clinicians describe cervical length instead. Effacement and dilation often progress together, but one may change before the other, especially in a first labour.
How a cervical examination works
With consent, a midwife or doctor inserts gloved fingers into the vagina to estimate dilation, effacement, the baby's position and how low the presenting part is. Measurements are clinical estimates, so two examiners may record slightly different numbers. Vaginal examinations are offered when the information could guide care; you can ask why one is recommended, discuss alternatives and decline it.
Why the numbers do not predict timing
A cervix can begin changing before labour, while active change usually occurs alongside increasingly regular, strong contractions. Being 1 or 2 centimetres dilated does not reveal whether labour will begin today or days later. Progress is assessed over time alongside contraction pattern, how you feel, membrane status and the baby's wellbeing—not by one number in isolation.
The stages around full dilation
The latent phase is when the cervix softens, thins and begins to open; it may last hours or days. In established labour, contractions become stronger and more regular as dilation progresses. The second stage begins at full dilation and ends with birth. The third stage is delivery of the placenta. Your individual pace may differ, and your team will recommend monitoring or support based on the whole situation.
